Initiating a Referral

To initiate the referral process, call IAH at (905)
337-8004. Our Home Co-ordinator or designate will initiate the referral
process which includes accessing medical records and assessment by physicians
on the admissions committee. Prospective residents and family members
will be visited by a Home Co-ordinator for assessment and will then be added to
the waiting list and prioritized according to the most urgent need for the next
available bed. Family members and, if possible, prospective residents are
asked to visit IAH in order to have an information session with a staff member
or volunteer. Equally important, these visits allow potential residents
and family members to ask any questions or to express any concerns that they
may have.
Admission Procedure
Prospective residents on our waiting list will be regularly contacted by our
Home Co-ordinator and we ask that we be kept informed of any major changes in a
prospective resident's circumstances. When contacted for admission it is
important that a prospective resident be admitted as soon as possible.
Residents are free to be discharged, if they wish, at any time.
Transfers may be made to a more appropriate setting, if the clinical
requirements of a resident are greater than those that can be provided at IAH
or if residents no longer meet the criteria for admission.
Criteria:
-
The applicant must be an adult (sixteen years or
older) and a resident of Halton or Peel
-
The applicant must be diagnosed with terminal
cancer.

The applicant will have a life expectancy of
three months or less.
-
The applicant must have a need of end of life
care as defined in terms of supportive and comfort measures.
-
The Applicant or family must be experiencing
circumstances that precludes the continued necessary care at home.
-
The Applicant must have an IAH Physician's
Referral & Consent Form, signed by their Family Physician or Designate.
-
An IAH application requires a complete and
detailed list of medical problems and social history as well as current
medications prior to admission.
